Caring for Older Adults

Our talented, experienced, and compassionate team of social workers collaborate and support older adults’ independence with a focus on their quality of life: health, comfort, safety, and happiness with a clear commitment to fostering their dignity. Together, we have extensive expertise and skills in the aspects of aging. Our approach is holistic and person-centered in how we offer seniors and their care partners support guidance, referrals, and direct services. 

What We Do

Services Offered

  • In-person assessment conducted to identify needs
  • Personalized goal development through collaboration; implement, monitor, and modify as needed
  • Frequent communication with those involved in older adult’s day-to-day life
  • Compassionate, caring emotional support for older adults and their families
  • Ongoing in-home visits with the care manager to continually work towards achieving identified goals
  • Caregiver support through education, advocacy, referrals to Adult Day Services (ADS), and connections for respite care and caregiver support groups
  • Coordination of internal referrals within CJE programs, such as Counseling, Benefits, Food Services, Legal Services, and Financial Assistance
  • Facilitation of referrals to professionals, including medical, home safety, transportation, money management, organization and decluttering, moving assistance, housing, and end of life care.
  • Coordination of plans for future care needs with older adult and family members
  • Admission or discharge guidance during transitions to and from hospital, rehab, and home

How to Get Started

Assessment

  1. One of CJE’s intake social workers will first conduct a scheduled phone interview to give preliminary recommendations and resource referrals. Finances will also be assessed for eligibility for the program.
  2. Once your eligibility is determined, a care manager will follow up with you directly and an in-home visit will be scheduled to meet with you.
  3. During our visit, we’ll talk with you to gain an understanding of your strengths and needs. You are welcome to include any family members or caregivers in our visit if you wish.
  4. We’ll then collaborate with you on a customized plan to meet your goals. We will provide advocacy, options and guidance to assist you throughout our partnership.

Care Management Services Eligibility

CJE Requirements

  • Age 60 or older
  • Reside within program boundaries (Northside of Chicago and the Near North, Northwest, and Far North Suburbs)

  • Have liquid assets under $300,000

If your assets exceed the above criteria, we offer care management services on a fee for service basis:

  • $150 per hour for care management, billed in 5-minute increments and invoiced monthly
  • $75 per hour for online shopping services, billed in 5-minute increments and invoiced monthly
  • We do not charge for mileage to/from older adult visits/appointments
  • We do charge for transportation/parking expenses while accompanying an older adult via any mode of transportation, and for any requested shopping expenses for medical, household, or other incidental needs. These charges are added to the monthly invoice.

Hoarding Task Force

Resources and Guidance

CJE’s Care Management Services provides education and resources as well as in-person, individualized support for older adults experiencing hoarding. Our Hoarding Task Force Guide offers comprehensive information on the definition, perspectives, and considerations for people who hoard including practical resources for addressing the situation. Our Resources Handout provides a list of emergency, case management, and resource contacts. We also partner with other community professionals or loved ones involved.
