CJE SeniorLife's Haag Pavilion for short-term rehabilitation is a comprehensive in-patient program that helps patients return to everyday life following hospitalization for an acute illness or injury. The moment a patient enters Haag Pavilion, we work with both the individual and his or her family members to create care, treatment and discharge plans that are unique to that person. With all private rooms, we make it easy for our patients to focus on recovery and rehabilitation.
Located at Lieberman Center for Health and Rehabilitation in Skokie, our talented interdisciplinary team includes physical, occupational, speech and creative arts therapists, dieticians, social workers and professional nurses.
What separates CJE SeniorLife's Haag Pavilion from most others is that Haag patients have the benefit of being seen by a physiatrist and a nurse practitioner Monday through Friday. Why is that a critical difference? Because the physiatrist and nurse practitioner can be the "eyes and the ears" for the attending physician who may not be able to come to Lieberman every day. The physiatrist/nurse practitioner model of care at Haag and Lieberman is not only unique in the world of skilled nursing centers. It's a smart approach that creates a positive and responsive environment in which older adults can thrive… review our outcome measures to see the difference. Our discharge planners can also help ease a patient's transition from rehabilitation to his or her return home by linking the patient to CJE's extensive network of programs and services such as Home Health, home-delivered means, Medicar and transportation.
