Responsibilities: Under the direct supervision of the Charge Nurse, in accordance with prescribed procedures and established quality care standards, provides direct personal and restorative nursing care to an assigned number of residents, at all times ensuring the safety an well-being of the residents.
Requirements: High school diploma or general education degree (GED) preferred. One to three months related experience and/or training. Must have certificate from an accredited school for Nurse Aide in State of Illinois and have passed any required competency testing.

RN - Float Pool-Part Time

Restorative Nurse

Responsibilities:
Responsible for overseeing and coordinating the restorative/rehabilitation needs of the residents at the Lieberman Centre Geriatric Health Centre.
Assure the highest professional standards for this program as well as conforming to the IDPH and IDPA guidelines.
Assesses the physical, psychological, psychosocial and spiritual dimensions of the resident and their families, as well as their educational and discharge needs in order to formulate nursing therapeutic restorative/rehabilitation regime for each resident is carried out.
Responsible for implementing and monitoring the restorative program in the nursing department
Requirements:
Bachelor’s degree in health care or related field pref’d
Currently licensed as a Registered Nurse in the state of Illinois

Develops, markets and sustains resident occupancy, as required. Recruits new residents via appropriate marketing activities. Assures that prospective residents, while still in the community, are aware of and utilize CJE services.

Interprets and properly articulates services and benefits to clients/members and the community at large, and assists in utilization of all resources on the telephone and in-person.

Responsibilities:
Assess the need for, create, plan and implement creative arts therapy services for the community members of Lieberman Center. Provide a combination of clinical, wellness and community building services based on the needs of individuals and the Lieberman community at large.
Document services provided to residents in the electronic medical record, including but not limited to care plan and progress notes.
Work closely with Lieberman team members to strive for the best possible quality of life for the Lieberman community members. This includes participating in resident care conferences as needed.
Communicate with family members when appropriate about residents’ involvement in therapy.
Educate and consult with CJE staff about the creative arts therapies and other arts-based interventions with older adults.

Responsibilities:
Provides a comprehensive assessment of the client's emotional, physical, functional, financial and environmental systems in order to develop a plan of care.
Develops and implements a plan of care for each client which is based on client needs.
Reviews plan of care annually and as needed to evaluate need for interventions.
Is knowledgeable about CJE services as well as community resources. Studies, learns and uses Agency procedures for providing service to clients. Interprets Agency's services to applicant/clients/community, creatively and effectively links client's needs with resources, and assists them in the utilization of all available resources.

Requirements:
Graduate of an accredited four year college or university with a degree in gerontology, health, nursing, social service or other related area. Minimum of two (2) years of experience in a human service field preferred; or,
Graduate of a master's level professional program with a degree in social work or nursing with one year of experience in a human service field.
Knowledge of both aging process and human behavior preferred.
Willingness and ability to travel to and from client/family/caregiver/community appointments as needed.

Sorts, handles, and processes U.S. mail and interoffice mail.
Photocopies and finishes a variety of documents, ranging from simple to complex.
